Please contact Nurse Bennett at cobennett@dallasisd.org to report any medical illness, physician notes for gym and recess, or other health-related concerns.

To ensure all students remain healthy and safe, if your scholar is currently taking medications, prescribed or over the counter, and will need them during the school day, please fill out a medication form for EACH medication.

This form is for ALL medications including over the counter medications.

Nurse Bennett is not allowed to administer ANY medication without a form that has both parent and doctor's signatures.

Once you have the form filled out and signed, it should be brought to the clinic with the medication.

The medication MUST be in the prescription bottle with a matching prescription label or the original container from the store.

Nurse Bennett will not be able to process any medication in a Ziploc bag or labeled incorrectly.
