If you have purchased items on behalf of the Sudie PTA, please complete the following form for reimbursement.  Please note that sales tax is not reimbursable  so ask your committee member for the tax exemption form.  Requests must be submitted within 30 days of purchase.  Once approved, the requester will receive a confirmation email and a check will follow in the mail shortly thereafter.
